Switching From Another Provider

If you're joining Verge after receiving treatment elsewhere, you're taking the right step—continuing your care with a provider that offers ongoing support, direct shipping, and personalized treatment. This guide explains what to expect during the transition.

It's Not a Direct Transfer

Switching providers is not simply continuing your previous prescription with a new name attached. Even if you were previously prescribed the same medication, our licensed medical providers must:

  • Review your complete medical history

  • Confirm your prior treatment details

  • Determine a safe and appropriate plan for you

This is a standard safety requirement for all telehealth providers. Your health profile, treatment history, and current needs are all factors in determining your plan.

What You'll Need to Provide

To ensure a smooth transition, you may be asked for:

  • Previous prescription details – medication name and formulation

  • Current or most recent dose – the amount you were taking

  • Pharmacy or provider information – where you received your medication

  • Treatment timeline – how long you've been on treatment

In some cases, documentation may be required (such as a prescription label or medical records) to verify your reported information.

Why Documentation Matters

Your provider needs to verify:

  • What you were actually prescribed

  • That your reported dose is accurate

  • That continuing treatment is medically appropriate

Without verification, your provider may start you at a lower dose or delay approval until more information is provided. This is for your safety.

Your Dose May Change

Even if you were stable on a certain dose before, your new provider may:

  • Start you at a lower dose

  • Adjust your dosing schedule

  • Modify your treatment plan

This is normal and based on:

  • Safety standards – providers take a careful approach when assuming care from another practice

  • Concentration differences – medications from different pharmacies may have different formulations

  • Gaps in treatment – if some time has passed since your last dose, your provider may restart you conservatively

Understanding Concentration Differences

Medications from different pharmacies may have different mg/mL concentrations. This means:

  • Your units (what you inject) may change

  • Even if your actual dose (mg) stays the same

For example, if your previous medication was 2 mg/mL and your new medication is 4 mg/mL, you would inject half the volume to get the same dose.

Always follow your new instructions—not your previous ones. Do not assume the units are the same. Using the wrong volume could result in an incorrect dose.

If There Was a Gap in Treatment

If you haven't taken your medication recently, you may not resume your previous dose. Instead, you may need to restart at a lower level. This helps reduce the risk of side effects and allows your provider to reassess your tolerance.

Gaps in treatment are common reasons for dose adjustments. Be honest about your timeline so your provider can make the safest recommendation.

What the Provider Evaluates

During your transition, your provider reviews:

  • Your prior dose and how you responded

  • Any side effects you experienced

  • How long you've been on treatment

  • Whether there was a gap since your last dose

This information determines your starting point with Verge.

Common Misunderstandings

If you think...

The reality is...

"I'll be continued at the same dose automatically"

Not guaranteed. Your provider makes an independent medical determination.

"My old instructions still apply"

They do not. Always use the new instructions provided with your Verge prescription.

"My units should match what I used before"

Not necessarily. Concentration differences may change the volume you inject.

"I can skip the medical questionnaire since I've done this before"

No. A new submission is required for every patient to ensure safe, compliant care.

How to Make the Transition Smooth

  • Provide accurate information – be complete and honest about your history

  • Upload documentation promptly – if requested, send records quickly to avoid delays

  • Respond quickly to follow-up questions – your provider may need clarification

  • Be open to adjustments – starting at a different dose is normal and safe

When to Contact Support

Reach out if:

  • You're unsure what information to provide

  • You don't have access to prior records

  • Your dose seems different than expected

  • You have concerns about restarting treatment

Summary

Switching from another provider requires a new medical review. Here's what to keep in mind:

  • Your treatment is not automatically continued at the same dose

  • Documentation may be required to verify your history

  • Your dose may be adjusted for safety

  • Instructions may differ due to concentration changes

  • Gaps in treatment may result in a lower starting dose

The goal is simple: ensure your treatment continues safely and correctly—based on your current situation, not just your past prescription.
